IMPORTANT INFO – OSU BUCKEYE INVITATIONAL & MEDICATIONS

Hello Band Families:
With the upcoming overnight trip to Columbus for the Buckeye Invitational I wanted to send out some information regarding medication for students.  Per district policy, students are not permitted to carry or administer their own medication while on a school trip. This includes all prescription and any over the counter meds such as Tylenol, Motrin, allergy medications, cold remedies, etc.   We have medication trained staff that will be responsible for administering these meds while on the trip should they be necessary.  Parents that are accompanying the band will be permitted to give medications to their own children.  Students with inhalers may carry them with the proper district authorization.
The forms for medication are on the Music Mentors website.  These forms need to be filled out by a physician and returned to Mr. Poremba by September 22 for students that are needing medication.  All meds must be turned in by Wednesday September 26 in a sealed NEW container containing the medication.  We will not accept any open medications.  As always please let me know if you have any questions.
